Some items in our Registration Packet are only available in the office.    We recommend you obtain a packet prior to registration.

Parents will need to bring the following on the day of registration:
Note: To enter school in September of 2007, a child must reach his or her fifth birthday on or before September 30, 2007.   No child who was born after September 30th , 2001, will be admitted.

  A certified copy of your child's birth certificate or completion of a required affidavit.   School officials will enter the birth certificate number into the pupil's permanent school record and return the birth certificate to the parent or guardian.
             Social Security Number
             Proof of Residency
             A)Deed for residential property or contract on a residential property in Prince William County
             B)Two of the following Items
             Valid Driver License
             Car registration
             Utility bill or utility hook- up paperwork
             Personal property tax bill
             County or town car sticker receipt
             Other bills
             Voter registration
             Other documentary evidence of similar reliability.

A certificate of physical examination for students entering grades kindergarten through five. Proof of a physical examination performed within one year of public school entrance must also be provided before school begins.   Physical examination forms are available from the main office of each elementary school.

The State Health Commissioner has clarified through a memorandum to the Department of Education, dated December 21, 2001, that a "comprehensive physical" includes (a) a screen for iron-deficiency anemia, or (b) hemoglobin, or (c) a hematocrit and a urinalysis.   The Virginia Department of Education and the Virginia Department of Health require documentation on the MCH 213D (Preschool Entrance Physical Examination and Immunization Form) of a hemoglobin or hematocrit within the previous twelve months, or documentation of an iron-deficiency anemia risk assessment as well as a urinalysis.

Immunization Record:
  Documentary proof of adequate immunization against Diphtheria, Polio, & Tetanus (DPT); Mumps, Measles & German Measles (Rubella) (MMR); whooping cough; Hepatitis B vaccine; and Chicken Pox (Varicella).  
             The following are the dosage/date requirements for all students:
             Two doses of measles vaccine (one dose received at 12 months of age or older and second prior to entering kindergarten)
             One dose each of the Mumps and Rubella vaccines received at 12 months of age or older (usually given as MMR)
             Three doses of DTP; one dose of DPT vaccine must have been taken after age four.  
             All susceptible children (those who have not had chicken pox) born after January 1, 1997, are required to have one dose of varicella not earlier than the age of 12 months.
             Students born after January 1, 1994 must present proof of having received three doses of hepatitis B vaccine with at least four months' time between the first and third dose.
             Children will NOT be allowed to start school without proper immunizations.
 
             A tuberculosis screening test is required for students who have been out of the United States for a period of five or more consecutive months during the previous five years.
Children may be vaccinated at the Manassas or Woodbridge clinic of the Prince William County Health Department or through other medical professionals. Vaccinations that are required for entrance into school are free. The fee for the tuberculosis test is $16.00. Parents should bring a copy of their child's shot record with them.

The Manassas clinic, located at 9301 Lee Avenue, Manassas, is open for immunizations from 8:30 to 11:00 am and 1:00 to 3:30 pm on Mondays and 8:30 to 11:00 am and 1:00 to 3:30 pm. on Tuesdays. Tuberculosis screening takes place on Tuesdays from 8 am. to 10 am. The telephone number is 703- 792-6300.
The Sudley North office of the Manassas clinic, located at 7751 Ashton Avenue, Manassas, gives immunization shots on Wednesdays from 8:30 to 11:30 a.m. and 1:00 to 3:30 p.m Call 703-792-4700 for the date and time physical examinations are offered at the Sudley North location.

The office hours of the Woodbridge clinic, at 13792 Smoketown Road, Woodbridge, are 1:00 pm to 3:30 pm. on Monday, 8:30 to 11:00 am and 1:00 to 3:30 on Tuesday,8:30 to 11:00 am and 1:00 to 3:30 pm on Thursday.
Tuberculosis testing takes place on Monday 8:30 to 10:00 am .The telephone number is 703-792-7300.
