Medications at School
All medicine (over the counter or prescription) administered at school must have a medication authorization form on file in the clinic. Prescription medicine must have a physician's signature on the form before medicine can be administered. All medicine must be in its original container, with the child’s name on it. The form and medicine must be brought to school by a parent or other adult, and will be kept in the school clinic.

IMMUNIZATIONS REQUIRED FOR SCHOOL ADMISSION
DTP or DtaP : At least 3 doses. ( One of these must be after the 4 th birthday).
Polio : At least 3 doses. ( One of these must be after the 4 th birthday ).
Measles, Mumps, Rubella : 2 doses. (1 STdose after 12 months of age, the second before Kindergarten).
Hepatitis B Vaccine : Series of 3 doses required for all children born on or after January 1, 1994 .
Chicken Pox or Varicella; All susceptible children born on or after January 1, 1997 , given one dose after 1 st birthday.
Haemophilus Influenzae Type b (HIB) : Required only for children entering school or day care at 30 months of age.
(All rising 6th graders must have Tdap shot at 11 years of age).
