Welcome

We hope the following information will help you in registering your child.

THE FOLLOWING INFORMATION MUST BE PROVIDED TO THE OFFICE PRIOR TO THE FIRST DAY OF SCHOOL:

Proof of Residence in the Old Bridge school boundaries.   The parent or guardian must provide proof of residence by providing one of the following documents: a deed for residential property or contract on a residential property, a current rental or lease agreement for residential property, or a notarized Affidavit of Residency.  

Additionally, two of the following showing the residence address must also be presented to school staff: a valid driver's license, car registration, utility bill or utility hook-up paperwork, personal property tax bill, county sticker receipt, voter registration, or other documentary evidence of similar reliability.

Birth Certificate - original or certified copy (we need only to record the birth certificate number and verify the birth date and student's name). An affidavit affirming birth may be completed and notarized.

Minimum Immunization Requirements

Two doses of MMR - one dose received on/after 12 months of age; the second dose prior to entering kindergarten.

Three doses of DTP - one dose received after the 4th birthday unless 6 doses prior to 4th birthday

Three doses of OPV - one dose must have been given after the 4th birthday unless the child received four doses prior to the 4th birthday

Three doses of Hepatitis B - 3 doses required  

Varicella - all susceptible children born on and after January 1, 1998 must have one dose no earlier than age 12 months.

Other Requirements

Physical Examination - required for all students entering kindergarten through fifth grade. A copy of a physical examination completed within one year of public school entrance must be provided. School entrance health forms are available in the Office.

Grades, Test Scores - from previous school

Student Transfer Form - required if student does not live within Old Bridge school boundaries but has a babysitter or daycare center, that is within the Old Bridge school boundaries. Transfers must be approved by the receiving principal and often depends on student membership.

A registration packet must be completed for every student entering Old Bridge. The best time to register is between 9:30 a.m. to 2:30 p.m. Please contact the Office for information regarding the necessary forms to be completed.

Other Information

A child who has reached his or her fifth birthday on or before September 30 must be enrolled in kindergarten unless the parents or guardians notify the school that they do not want the child to attend school.

A child who will be six years old on or before September 30 must attend school.

Students who do not have all of the immunizations and a physical examination will not be allowed to attend school. However, students may be conditionally enrolled if they provide proof of having received at least one dose of the required doses within ninety days and a physical examination recorded on the state form, School Entrance Health Form.

Objections based on religious or medical grounds may be discussed with the principal.
Immunizations may be received at a clinic of the Prince William County Health Department or through other medical professionals.

Kindergarten Registration

Kindergarten registration for next year will be held Wednesday, April 9 and Wednesday, April 23 from 9:30 a.m. – 2:30 p.m.  To enter school in September, 2008, a child must turn five by September 30, 2008.

Bring the following to Old Bridge when registering:

  1. Proof of Residence in the Old Bridge school boundaries must be provided with one of the following:  a deed for residential property or contract on a residential property, or a notarized Affidavit of Residency.  Additionally, two of the following showing the residence address must also be presented to school staff:  a valid driver’s license, car registration, utility bill or utility hook-up paperwork, personal property tax bill, county sticker receipt, voter registration, or other documentary evidence of similar reliability.
  2. Birth certificate – original or certified copy (we need only to record the birth certificate number and verify the birth date and student’s name for children born in the United States).  An affidavit affirming birth may be completed and notarized.
  3. Child’s social security number
  4. Physical examination – A comprehensive physical examination completed within one year of public school entrance must be provided.  Physical examination forms are available in the Office.
  5. Immunizations requirements:
    • Two doses of MMR – one dose received at 12 months or older and the second dose received prior to entering kindergarten
    • Three doses of DTP – one dose received after the fourth birthday
    • Three doses of OPV – one dose must have been given after the fourth birthday unless the child received four doses prior to the fourth birthday
    • Three doses of Hepatitis B
    • Two doses of Varicella - all susceptible children born on or after January 1, 1997 must have one dose administered at age 12 months or older, second dose prior to entry into Kindergarten
