ENROLLMENT REQUIREMENTS

Per Regulation 366 – 1

Child must be 5 years old by September 30, 2008 to attend Kindergarten.

____Proof of Residence - (Ex. Deed, Rental or Lease agreement for residential property, or Notarized Affidavit of Residency)

____Two additional documentations proving address (Ex. Valid Driver’s license, Car registration, Utility bill,  Personal Property tax bill, County car sticker receipt, other bills, voter registration)

____Physical Form (dated after 9-03-07) 

____Immunization Record (3 DTP/3 OPV –one after 4th birthday) (1st MMR after 1st birthday /2nd one after 4th birthday) (Hepatitis B – 3 doses)

____Birth Certificate (born on or before 9-30-03)

____Social Security Card

____Custody Papers (if applies)

____Registration Forms

____Transfer Request (if applies)
