Child(ren) entering kindergarten must be 5 yrs. old by Sept. 30. In order to register your child(ren) into a Prince William County School, you must have the following: Physical examination (including dates of current immunization) by a licensed physician dated no earlier that 12 months prior to entry into public school.  3 DPT (1 dose after 4th birthday)  3 ORAL POLIO (1 dose after 4th birthday)  MMR - 1 after 15 months and 1 after 4th birthday (students entering kindergarten and first grade for the first time in Prince William are required to have a second MMR shot that has been given after the 4th birthday. Hepatitis B - any child born after January 1, 1994 is required to have this series of shots. Certified copy of the child's birth certificate & a copy of the child's social security card.  Proof of residence- Deed or contract on residential property, or lease agreement or homeowner's contract with home address with parent/guardian name, or a notarized Affidavit of Residency will be accepted. IN ADDITION, you must have 2 of the following: Driver’s license Car registration Utility bill or utility hook-up paperwork Personal property tax bill County or town car sticker receipt Other bills Voter Registration Other documentary evidence of similar reliability
