Please complete this form and return it to the school location that you are requesting. Requests for use of the schools can be done via the Internet. Web Requestor instructions are available in Word by clicking here or in PDF format by clicking here. Please follow the instructions carefully. Failure to follow the instructions step-by-step may result in your request not going through. You will receive e-mails telling you the status of your web request. Both the Community Use Payroll Information Form and the Community Use Monthly Payment Sheet are available in FS Direct for school personnel. Please be advised that PWCS restricts the use of new athletic fields to internal use only until at least late spring of the school's first year and, if conditions warrant, until the fall of the second year. This is imperative so the grass has sufficient opportunity to fully establish. Thank you. |
