The Prince William County School Division provides workers' compensation coverage to all employees (full time, part time, temporary, substitutes) through a self-insured program administered by VML Insurance Programs (VMGSIA) at P.O. Box 71420, Richmond, VA 23255 effective August 1, 2008. Any inquiries or contacts with reference to workers' compensation claims should be directed to VML at 1.800.963.6800 or fax number 1.804.273.0560.

WORKERS' COMPENSATION FORMS

All employees shall promptly report all work related injuries or illnesses to their immediate supervisor. Complete the Employee's Report of Accident (Attachment I) and the Supervisor's Report of Accident (Attachment II) and forward to VML Insurance Programs, P.O. Box 71420, Richmond, VA 23255 .

If you are seen by a doctor (see below panel of doctors), the Workers' Compensation Transmittal Sheet (Attachment III) should be presented at each doctor visit.

PANEL OF DOCTORS

Employees may seek medical attention, if necessary, and shall select one (1) of the authorized physicians or medical facilities as listed below.  The closest emergency facility may be used in an emergency situation. Once the emergency treatment is completed, a panel physician must be chosen for follow up care.

NOTE:  EMPLOYEES WILL BE RESPONSIBLE FOR ANY MEDICAL CHARGES IF THEY ELECT TO VISIT A FACILITY OTHER THAN THOSE DESIGNATED BELOW.

If an injury occurs that results in a blood/body fluid exposure, contact any one of the below physicians or facilities for medical treatment.  If the injury occurs after hours, seek medical attention at the Emergency Room.

PHARMACIES

Employees may obtain workers' compensation prescriptions through the Modern Medical pharmacy network.

Modern Medical is the pharmacy management provided for VML Insurance Programs. They have a pharmacy network in Virginia that includes more than 1,500 pharmacies in the major pharmacy chains and small local businesses.

The injured employee is provided with a first fill temporary prescription card when the injury is reported. This card is presented to the network pharmacy along with the prescription for the initial supply.

LOST TIME

State law requires that the first seven calendar days for an injury, which includes weekends and holidays, will be charged against the injured employee's sick leave benefits. Upon approval of the claim, benefits will commence on the eighth (8th) day of disability and paid at the rate of two-thirds of the employee's gross average weekly wage (subject to maximum and minimum rates set by the Virginia Workers' Compensation Commission).  See Section D of Regulation 532-1.

NOTE TO TIME KEEPERS:  When recording disability due to a work-related injury, please record as sick leave, or in the event the employee has no leave benefits whatsoever, record as leave without pay until such time as the claim is approved by the VML Insurance Programs. You may not use the sick leave bank or donated leave for work-related injury leave. You will be notified by VML Insurance Programs once the claim is accepted as to how to record such period of disability.
