WELCOME TO ELIZABETH VAUGHAN. . . If your child has previously attended a Prince William County Public School at any time, you will only need to fill out a single transfer form you can obtain in the office.

If your child is new or is coming from a school outside of Prince William County, you will need to fill out the entire registration packet which you can get in the office or print now.

THE FOLLOWING INFORMATION MUST BE RECEIVED BEFORE YOUR CHILD CAN ENTER SCHOOL AND BE ASSIGNED TO A CLASS:

1. Birth Certificate – with raised State Seal {we need only to record the BC # and verify the birth date and birth state}. Copies are not accepted. If kindergarten, the child must turn five (5) years of age on or before September 30. If you do not have the child’s birth certificate, you will need to fill out a Birth Certificate Affidavit form and have it notarized.

2. Social Security Number – Copies are not accepted. (We only record the number.)

3. Physical Form – students must have proof of physical examination performed within one year of entrance and before school begins. Physical must be signed and dated by a physician. Also, hemoglobin and urine testing must be completed and dated by the physician.

4. Immunization Record – student must have the following completed:

3 DTP’s – 3rd after child’s fourth birthday
3 OPV’s – 3rd after child’s fourth birthday
2 MMR’s – 2nd after child’s fourth birthday
3 HEP B – for children born on or after January 1, 1994
1 Varicella – if child has not had chickenpox

5. Proof of Residence – This can be a utility bill (water, phone or etc.), a lease or a
notarized letter from the person you are residing with.

6. If a custody situation exists, both parents have a right of access to the student
and their records UNLESS we have a copy of the court document giving custody
to one parent.

BIENVENIDOS A LA ESCUELA ELIZABETH VAUGHAN. Esperamos que la siguiente información se de ayuda. La siguiente es una lista de los varios requerimientos para la matriculación de su hijo/a. Si su hijo previamente ha asistido a una escuela elementar en el condado solo tendrá que llenar una forma de transferir. Si su hijo/a viene de una escuela fuera del condado de Prince William, usted necesitara llenar el paquete de matriculación que recibirá en la oficina de la escuela.

NECESITAMOS LO SIGUIENTE ANTES DE ASIGNAR A SU HIJO/A A UNA CLASE:

1. Certificado de Nacimiento- necesita ser certificado original(es solo para verificar la fecha, el lugar de nacimiento, y el numero del certificado) No podemos aceptar copias. Si el niño va entrar a Kinder, debe tener 5 años ya para el día 30 de Septiembre de ese mismo año. Si no tiene certificado de nacimiento, usted necesitara llenar un "Birth Certificate Affidavit" y este necesita ser notariado.)

2. Numero de Seguro Social- No aceptamos copias.(Solo necesitamos verificar el numero)

3. Forma de Físico- debe de haber prueba de un examen físico. Este examen debe de haberse recibido entre un año antes de la entrada del niño. Este es necesario ANTES de poder aceptar al niño. El físico debe ser firmado por el doctor y debe tener la fecha. También son necesario el examen de orina y de la hemoglobina.

4. Vacunas - El estudiante debe tener las siguientes:

3 DTP’s – 3ra despues del 4to cumpleaño
3 OPV’s – 3ra despues del 4to cumpleaño
2 MMR’s – 2da despues del 4to cumpleaño
3 HEP B – para ninos que nacieron en o despues de 1 de enero 1994
1 Varicella –si el niño no a tenido varicella

5. Prueba de Residencia - Esta puede ser una factura de agua o teléfono etc. También puede ser un sustantivo de la casa o apartamento donde vive con su nombre y dirección o una carta notariada de la persona(s) con quien vivan.

6. Si hay una situación de custodia, los dos padres tienen acceso a su hijo/a a menos que tengamos una copia de la corte que solo le de acceso a un padre/madre/guardián.
