Request a 1095-C Health Care Coverage Form
As of January 2025, the Affordable Care Act requires certain employers to provide the 1095-C form to full-time employees and their dependents upon the employee’s request. The 1095-C form contains detailed information about your health care coverage.
If you would like to receive the 1095-C form, you must submit a request for one. The 1095-C will be mailed to the employee using U.S. mail within 30 days of this request. Although you may use the information on the forms to help complete your tax return, these forms should not be attached to your return or sent to the IRS. Taxpayers can prepare and file their returns using other information about their health insurance.
