Filing a Complaint of Discrimination or Harassment Involving a Protected Category

To file a complaint of discrimination or harassment involving a protected category, you may put your concerns in writing by completing a complaint form. A complaint should be filed immediately following the event giving rise to the event but no later than one year after. Once completed, the form should be forwarded to:
