All PWCS students in grades K-12 are eligible for Hazel Health services.
- How can Hazel Health's services support my student?

Hazel Health will offer immediate essential mental health services to PWCS students, at home, at no cost to your family.
Hazel health therapists can help students with:- Anxiety, depression, and stress
- Mood and behavior changes
- Grief and loss
- Social skills, bullying, and peer relationships
- How can students access Hazel Health services?

There’s a multi-step process to access Hazel Heath Services.
First, parents or guardians (and students over age 18) must opt in their student via ParentVUE. Student information is then shared with Hazel Health within one business day (typically overnight).
Second, parents/guardians (and students over age 18) must complete the Hazel Health consent form.
After completion of the Hazel Health consent form, parents/guardians can schedule the first appointment through Hazel Health.

- When can Hazel Health telehealth appointments be scheduled?

Telehealth appointments are available 7 a.m. - 7 p.m., Monday through Friday.
Please note that Hazel Health telehealth appointments do not take place at school.

Following the completion of the opt-in form in ParentVUE, PWCS will share student and parent/guardian information with Hazel Health to verify student enrollment in PWCS. The following information will be shared:

All parent-provided information will be securely entered into Hazel Health’s HIPAA and FERPA-compliant system by the parent.
- Will school staff know that my student is receiving mental telehealth services?

No information about a student’s telehealth services is released to schools or PWCS.
Hazel Health complies with all HIPAA and FERPA requirements.
Hazel Health does not share information about a student’s telehealth services with schools, school staff, or PWCS.
Parents and guardians can choose to share information directly with school staff if they believe that it will help the school support their student.
- What should I do if my student has a mental health emergency?

Hazel Health services are not a replacement for emergency services.
If a child needs immediate medical attention or in case of a life-threatening emergency, call 911 (TTY dial 711).
For an emergency mental health assessment, visit the nearest emergency room.
- What is the cost of Hazel Health’s telehealth sessions?

All telehealth services will be provided to PWCS students by Hazel Health at no cost to families, regardless of their ability to pay or insurance status.
- Why does Hazel Health ask for insurance information?

Hazel Health asks for health insurance information as part of the consent to care; however, all students can receive services regardless of their insurance status. If insurance information is provided, Hazel Health will attempt to receive reimbursement from the insurance company for services provided. Families are never charged for Hazel Health sessions. Any patient responsibility (e.g., copay, coinsurance, deductible) on an explanation of benefits (EOB) is covered by PWCS’ contract with Hazel Health.
- Can my child see a Hazel Health therapist if they also receive care from another provider?

In alignment with mental health industry standards, Hazel Health cannot duplicate services already provided by another provider. Please let your Hazel Health therapist know if you are receiving any other mental health services during your intake visit so that they can determine whether it is clinically appropriate to pursue Hazel Health’s program.
- Are Hazel Health telehealth services available in languages other than English?
