Instructions for Accessing Care through Hazel Health
Parents and guardians (and students over age 18) must begin the process of accessing telehealth care through Hazel Health for their student.
Step 1: Complete the opt-in form in ParentVUE
Parents and guardians must complete an opt-in form in ParentVUE to allow PWCS to share student and parent/guardian information with Hazel Health to verify student enrollment in PWCS.
It may be easier for parents and guardians to complete this step on a computer.
- Log in to ParentVUE.
- Select the student that you would like to opt-in.
- Click Student Info then click Edit Information.
- Go to the Hazel Health Opt-In section.
- Type your name in the box below Parent Signature.
- Click Save Changes to provide PWCS with permission to share data with Hazel Health for the selected student.
- Repeat Step 1 if you would like to complete the opt-in form for more than one student.
Please allow one business day for your student’s information to be shared with Hazel Health. This typically happens overnight.
Step 2: Register your student with Hazel Health
- Visit the Hazel Health website for PWCS and complete the consent form.
- The parent/guardian must complete the Hazel Health consent form before a student can access services.
- After completing the consent form, a parent/guardian must create an account with Hazel Health.
- The parent/guardian will be prompted to submit a referral for services when creating an account.
Step 3: Initiating services with Hazel Health
- A referral must be completed and submitted to initiate services.
- Once a referral is submitted, the Hazel Health enrollment team confirms that the parent/guardian consented to telehealth services and schedules an intake appointment for your student.
- Your student participates in regularly scheduled telehealth sessions.
Know before you begin services
- A parent/guardian, in conjunction with Hazel Health, schedule each telehealth appointment for their student.
- Hazel Health telehealth appointments do not take place at school.
- Please note that Hazel Health requires a parent, guardian, or trusted adult to be at home during telehealth appointments.
