The deadline to meet the school physical requirements for students in Pre-K through 5th grade for the upcoming school year is August 21, 2026. If proof of a required school physical is not provided to the school by the deadline, the student will not be permitted to start school on Monday, August 24, 2026, until a school physical is provided.
School physicals must be dated after August 24, 2025. School physicals dated before August 24, 2025, will not be accepted. Learn more about school physical requirements in Regulation 723-5.
Download the required school physical health form (PDF). This form must be completed by the healthcare provider administering the school physical.
The completed school physical health form must be returned to your student’s school indicating that your student has received a school physical. School physical health forms must be delivered in person to the school's office.
If your student cannot receive a school physical for some reason, the healthcare provider must give you a medical exemption statement to provide to your student’s school.
Prince William County Public Schools is partnering with local healthcare facilities to offer no cost school physical clinics for students in Pre-K through 5th grade.
